Print the registration form below (right click, choose "Print Picture") or use the contact form to request an emailed copy of the class registration sheet. Fill out the form and return with your check made payable to Mid-Ohio Fiber Fair to:

Your classroom seat will not be secured until payment is received.
Materials Fees are to be paid directly to the teacher on the day of class.
Cancellations may be made IN WRITING by August 5, 2012.
NO REFUNDS OR CANCELLATIONS after August 5, 2012.
If a class is cancelled due to attendee minimums not being met, a refund will be made.
Registrations must be received by August 3, 2013.
You will receive a confirmation email when your paperwork and payment is received and you are registered for your class(es). Pick up your student packets, which include your admission armband(s), at the admission desk when you arrive at the fair.
